Cingulo-Insular Structural Alterations Associated with Psychogenic Symptoms, Childhood Abuse and PTSD in Functional Neurological Disorders

Objective Adverse early-life events are predisposing factors for functional neurological disorder (FND) and post-traumatic stress disorder (PTSD). Cingulo-insular regions are implicated in the biology of both conditions and are sites of stress-mediated neuroplasticity. Functional networks of motor inhibition in conversion disorder patients and feigning subjects

The neural correlates of motor inhibition leading to paresis in conversion disorder are not well known. The key question is whether they are different of those of normal subjects feigning the symptoms. Thirteen conversion disorder patients with hemiparesis and twelve healthy controls were investigated using functional magnetic resonance tomography under conditions of passive motor stimulation of the paretic/feigned paretic and the non-paretic hand. Previously referred to as conversion disorder, FND is a disorder of the communication between mind and body resulting in the experience of neurological symptoms incompatible with neurological or medical diagnoses. Hysterical paralysis is a rare variant of conversion disorder. Its diagnostic can be difficult, and organic causes of paralysis must be ruled out. We report here a case of hysterical hemiplegia that was attributed to occupational-linked stress factors, and was consequently considered as an accident at work.
